    The ref did

Never given it much thought, but home captain tosses, visiting captain calls - seems to be the way I do it?

The few times I've offered a coin to a captain they've stared at me. Seems to be normal in the Netherlands that the referee tosses the coin, visiting captain calls.

I normally toss the coin and the home captain calls. Never been told anything different.

DUTIES OF THE REFEREE BEFORE THE MATCH
    3. The referee organises the toss. One of the captains tosses a coin and the other captain
    calls.
The winner of the toss decides whether to kick off or to choose an end. If the
winner of the toss decides to choose an end, the opponents must kick off and vice versa.

I have the same experience. I think both methods happen in England (as they evidently do in RWC )
