    the WR clarification is not even useful at elite level because - AIUI - players come on and off the field supervised by the officials, not by the ref. The ref is just told.

    well that's the nub of this isn't it -- when you blow whistle for PK ball is still alive (as they can quick tap). The ball only goes properly dead (for a replacement) when you say it's dead.

    I think it means you treat it as if the defending player had touched down - so if brought into in goal by attackers = 22m DO, if by the defenders = 5m scrum
